(申辦居留證、健保卡使用)

*2-inch passport-style photo

(To apply for the resident certificate and an NHI Card.)

• 最近2年內所拍攝。
• 彩色、脫帽未帶有色眼鏡五官清晰、不遮蓋，相片不修改，足資辨識人貌，直4.5公分橫3.5公分，人像自頭頂至下顎之長度不得小於3.2公分及超過3.6公分，白色背景之正面半身薄光面紙照片。
• 相片未經修改編輯且不得使用合成照片。

• The photos taken within the last two years. 
• 2-inch, upper body, front-facing, photo with no hat, no color-lens glasses, with clear facial features taken recently.
• In consideration of clarity of identity, the photo may not be edited or superimposed. 

*體檢報告

*Physical examination report

• 辦理簽證時使用的合格體檢報告。（國外之認可醫院名單）
• Qualified physical examination report used for visa application. (The list of authorized hospitals overseas)
• 港澳生：請準備針卡 (預防接種疫苗紀錄本)，並於入台後進行體檢。（外國人健檢指定醫院名單(國內)）

父母親身分證影本

Copy of parents' ID card

• 可用於電信公司申辦手機門號與銀行帳戶申辦。
• It can be used to apply for mobile phone numbers and bank account.

清寒證明

(申請清寒助學金、清寒健保需求者)

Underprivileged certificate

(To apply for a reduction in National Health Insurance premiums and grants for overseas chinese students in financial need.)

• 可用於申辦全民健保清寒僑生減免、清寒僑生助學金等。
• 須由留臺校友會、前一學程畢業學校、同鄉會、僑居地政府機構或僑委會指定之保薦單位所開立(非個人)，相關單位請參閱附件。
• Can be used to apply for a reduction in National Health Insurance premiums and grants for overseas chinese students in financial need.
• It must be certified by the Taiwan Alumni Associations, the graduating school of the previous program, the hometown association, or a recommendation unit authorized by the OCAC. (About the list)
